    Which Form Do You Need?

    The form depends on how old your current passport is and how old you were when it was issued:

    Free: Form DS-5504

    Use this if your passport is less than 1 year old. Name changes within the first year are free. You must submit your original passport and name change document.

    $130: Form DS-82 (Renewal by Mail)

    Use this if your passport is less than 15 years old and you were 16 or older when it was issued. This is the most common scenario for adults updating after marriage or divorce.

    $165: Form DS-11 (New Application)

    Use this if your passport is 15+ years old, you were under 16 when it was issued, or your passport was lost or stolen. Must apply in person at a passport acceptance facility.

    2×2 inch color photo, white background, taken within 6 months. Available at CVS, Walgreens, UPS stores, and most post offices for $10–$15.
